This website is not a patient portal and does not hold your medical records. How we use and protect your health information as a patient is described in our Notice of Privacy Practices, as required by HIPAA. You can ask for a copy at the front desk or by contacting us using the details below.

Please do not send medical details, symptoms or insurance numbers through this website's contact form. If you have a medical emergency, call 911 or go to the nearest emergency room.

When you use our contact form, we collect your name, email address, phone number (optional) and message. We use it only to respond to you. Submissions are stored in our secure website database, and our staff receive an email notification so they can reply.

With your submission we also save how you found our website (for example, “Google search” or the name of a campaign link), so we know which outreach is helping patients reach us. This is kept with your message and is not shared with analytics providers.

Our “Schedule Appointment” buttons open a booking window provided by Tebra, our practice management and scheduling provider. Information you enter there goes directly to Tebra on our behalf and is protected as health information. Our website and our analytics do not see what you enter in the booking window.

We use Google Analytics to understand how the website is used, so we can improve it. It tells us things like which pages are visited, how visitors arrived (for example, from a search engine), the type of device and browser used, approximate location (city level), and how often visitors use the buttons to call us, get directions, view the map, email us, schedule an appointment or send the contact form. Google Analytics uses cookies to tell visits apart.

We have set it up to limit what is collected:

• We never send your name, email address, phone number, the contents of any form, or any health information to Google Analytics.
• Analytics runs only on our public pages and never inside the appointment booking window.
• Google signals and ad personalization are turned off. We do not use analytics data for advertising, and we do not sell or share it for advertising.
• Analytics data is kept for no more than 14 months.

You can opt out in several ways: block or delete cookies in your browser settings, install the Google Analytics opt-out browser add-on, or turn on Global Privacy Control in your browser, which we honor by not running analytics for you. Maps on our site are provided by Google Maps and load only when you choose “Show map”. Links to directions, our Google listing and reviews open Google's own websites. When you use them, Google's privacy policy applies.

Like all websites, our hosting provider automatically receives technical information such as your IP address and browser type when you load a page. It is used to deliver and secure the site.

We do not sell personal information. We share information only with service providers that help us run the website and respond to you (website hosting, our database, email and appointment scheduling), each limited to that purpose, or when required by law.

The website is served over an encrypted (HTTPS) connection, and access to contact form submissions is limited to authorized staff. No method of transmission over the internet is completely secure, which is another reason not to send medical details through the contact form.

This website is intended for adults, including parents and guardians seeking care for their children. We do not knowingly collect information online from children under 13.

We may update this policy as our website changes. The effective date at the top shows when it was last revised.
